2012-13 Enrollment

For 2012-13 9th graders:

  • Sibling Preference Registration runs from October 1, 2011 – October 28, 2011
  • Registration/Presentation Day begins at 9am Saturday October 29, 2011
  • Pittsburgh Public School District Registration runs from October 29, 2011 – December 1, 2011
  • Open Registration for all PA residents starts December 2, 2011

2012-13 Enrollment Form

Please print the CCHS Enrollment Form 2012-13 and fax or mail it back to us along with the documents listed below. Complete* enrollment forms will be placed in line according to the date received within the appropriate enrollment period.

*Please make sure you submit all the documents listed below with your enrollment form in order to complete the enrollment process for your child.

Documents required to complete the enrollment application process:

  1. First proof of residency (Driver’s license, State ID, Voter Registration Card)
  2. Second proof of residency (Utility Bill, Mortgage Deed/Lease)
  3. A copy of your child’s birth certificate
  4. Legal guardianship documents (If you are not the child’s biological parents)
